Sumario: | A middle-aged man has rapid, pressured speech and an elated affect. He has grandiose ideas about his talents, claiming he self-taught himself to play the violin and reports rapidly jumping from one activity to another with enormous energy and enthusiasm for two months. He often is silly in the interview, singing and making puns, jumping from topic to topic. He has grandiose ideas about his talents, claiming he gives violin concerts in the NY subways, “transporting people” who are listening, playing music that he composed. He is difficult to redirect. He reports decreased sleep, only needing three hours per night. He demonstrates poor judgment, reporting he told farmers how to farm, and tells the interviewer how he should redecorate his lobby. He reports great concentration but when asked about reading he say he does not have time for reading because he has concerts in his head and abundant, rapid thoughts about inventions to make. He describes his thinking as being as fast as “jet blasts.” He says he is unable to organize to write down his ideas. When asked about dangerousness, he jumps to talking about sex, saying he has much promiscuous sex. He demonstrates dangerousness, saying he insulted strangers who then hit him in the face. He also told of impulsive buying because money told him to buy objects. When asked what he meant about “money speaking to him,” he becomes defensive and begins rhyming (clang associations). He becomes tangential, often not making sense. He reports he has auditory hallucinations that comfort him and which he sees as a gift.
